To reduce waste and promote efficiency, the City of Oak Park is taking advantage of a law that allows us to make your IRS Form 1095-C available to you only upon request.

We are required to file this form with the IRS to show whether we offered you healthcare coverage in one or more months of each calendar year. If you recall, in the past, this form was mailed you in the first quarter of the year. However, pursuant to the law, we will not be mailing the forms this year.

While we are no longer automatically mailing the form to you, you are able to request a copy of your form at any time.
